This resource directory is for families/caregivers of children who have medically complex needs or long-lasting conditions that require extra health care and support services.
The information in this directory is provided to help you find resources to assist you with finding help for your child and family. The range of care/disability/health issues varies amongst children so the information may or may not pertain to your child.

After a family learns that their child is ill parents/caregivers typically begin to think about how they can best care for their child. The resources in this directory include agencies, service providers, advocacy organizations, and other resources to assist with providing a quality life for children with medically complex needs.

With locations in community districts throughout the five boroughs, Homebase remains the cornerstone of the City’s homelessness prevention efforts, crafting individualized assistance to meet the needs of each household. Among the services that may be offered are:
- Family or tenant/landlord mediation
- Household budgeting
- Emergency rental assistance
- Job training and placement
- Benefits advocacy (child care, food stamps, tax credits, public health insurance)
Bronx Independent Living Services (BILS) is a non-profit, community based organization dedicated to empower all people with disabilities to understand and exercise their civil and human rights in order to live fully integrated lives in mainstream society. BILS assists individuals by providing them the necessary tools to make informed decisions about their own lives through access to education, skills development, and access to the appropriate resources.

The Staten Island Center for Independent Living, Inc., is a non-residential resource center that offers services to individuals with disabilities of all ages, their families, significant others, teachers, employers, businesses, and those who believe that an individual with a disability has the right to take responsibility for his/her own life.
The center is unique in that it is primarily staffed and governed by professionals who have had a personal experience with a disability and believe that disability does not mean inability.

What constitutes a family with children?
DHS considers families with children to be the following households:
- Families with children younger than 21 years of age
- Pregnant women
- Families with a pregnant woman
Where do families with children apply for shelter?
All families with children must apply for shelter at:
Prevention Assistance and Temporary Housing (PATH)
PATH is open 24 hours per day, including weekends and holidays.
PATH processes applications during business hours (9 a.m. to 5 p.m.)

To report repairs that need to be made in a privately owned building a tenant should first attempt to notify the building owner, managing agent, or superintendent. If the condition is not corrected, the tenant should call the City’s Citizen Service Center at 311. 311 is open 24 hours per day, seven days per week (hearing-impaired tenants can call (212) 504-4115 via a Touchtone Device for the Deaf TDD).
To report repairs that need to be made in a New York City Housing Authority (NYCHA)-managed building:
Tenants in NYCHA buildings should report repairs to their management office and fill out a work ticket. For more information, visit NYCHA's website.
To report repairs that need to be made in a Housing Preservation & Development (HPD)-managed building:
Tenants should report apartment maintenance issues to (212) 694-2381. For repair emergencies after hours and on weekends, tenants should call 311 and ask for the HPD Central Complaint Bureau
