This resource directory is for families/caregivers of children who have medically complex needs or long-lasting conditions that require extra health care and support services.

The information in this directory is provided to help you find resources to assist you with finding help for your child and family. The range of care/disability/health issues varies amongst children so the information may or may not pertain to your child.

What you'll find

After a family learns that their child is ill parents/caregivers typically begin to think about how they can best care for their child. The resources in this directory include agencies, service providers, advocacy organizations, and other resources to assist with providing a quality life for children with medically complex needs.

  1. Consumer Directed Personal Assistance program (CDPAP) cdpaanys.org

    This Medicaid program provides services to chronically ill or physically disabled individuals who have a medical need for help with activities of daily living (ADLs) or skilled nursing services.
    Services can include any of the services provided by a personal care aide (home attendant), home health aide, or nurse. Recipients have flexibility and freedom in choosing their caregivers.
    The consumer or the person acting on the consumer’s behalf (such as the parent of a disabled or chronically ill child) assumes full responsibility for hiring, training, supervising, and – if need be – terminating the employment of persons providing the services.

    To be eligible for CDPAP, an individual must:

    Be Medicaid eligibile;
    Be eligible for home care services, personal care services, or private duty nursing;
    Require some or total assistance with one or more home care, personal care, or skilled nursing service;
    Have a stable medical condition; and
    Be self-directing or have a designated representative who is willing and competent to assume the roles and responsibilities that go with the program.

  2. DREAMMS Assistive Technology for Children with Special Needs DREAMMS.org

    Facilitates technology access for children with special needs.  Information clearinghouse and search facility.

  3. First Hand Foundation firsthandfoundation.org

    First Hand is a not-for-profit organization that provides funding for individual children (up to age 18) with health-related needs when insurance and other financial resources have been exhausted. Funding may be available for clinical procedures, medicine, therapy, prosthesis; equipment, such as wheelchairs, assistive technology equipment, care devices, or hearing aids; and vehicle modifications.  Lodging, food, gas, parking and transportation for families of seriously ill children who must travel during treatment are also among the needs which might be covered.

  4. Healing the Children htcne.org

    A non-profit, non-partisan, volunteer organization, whose purpose is to help children throughout the world receive medical care unavailable to them due to a lack of medical and financial resources or health insurance. The Domestic Kids Program provides medical treatment and assistance to children in our local communities who are unable to get these services due to a lack of financial resources or medical insurance. They have arranged for children to receive services such as dental, medical, and surgical care, medical equipment, prescription medications, medical equipment, physical therapy, speech therapy, etc.

    The Personal Care program provides home attendant and/or housekeeping services to Medicaid-eligible clients who are having difficulty with at least one or more activities of daily life, such as walking, cooking, cleaning, bathing and using the bathroom, and who are in stable medical condition.

    UCP Nassau Residential Program offers a variety of life style options for adults with developmental disabilities.  Individualized and personalized service is the primary mission of the program.     UCP NYC operates residential facilities in the Bronx, Brooklyn, Manhattan and Staten Island, and offers a variety of residential services for adolescents and adults with physical and developmental disabilities. UCP of NYC residential services span a range of care and support levels, from intermediate care facilities for persons with complex needs, to supportive apartments for those requiring minimal staff assistance.
