This resource directory is for families/caregivers of children who have medically complex needs or long-lasting conditions that require extra health care and support services.

The information in this directory is provided to help you find resources to assist you with finding help for your child and family. The range of care/disability/health issues varies amongst children so the information may or may not pertain to your child.

What you'll find

After a family learns that their child is ill parents/caregivers typically begin to think about how they can best care for their child. The resources in this directory include agencies, service providers, advocacy organizations, and other resources to assist with providing a quality life for children with medically complex needs.

    AHRC Nassau a family governed organization, is dedicated to enhancing the lives of individuals with intellectual and developmental disabilities and their families

    AHRC Nassau operates the largest residential program in New York State with over 75 homes and 54 supportive apartments. AHRC is currently serving individuals in every community throughout Nassau County. Staff members provide 24–hour support for residents, while encouraging individuals to achieve greater levels of independence.  Individuals must be 18 years of age or older, in order to live in one of AHRC’s residences.

    Formerly known as the Brooklyn School for Special Children (BSSC), Community Residential Opportunities for Handicapped (CROH), and Training & Rehabilitation in Useful Service (TRIUS). Based in Brooklyn, New York, we provide unique and individualized services to children and adults with disabilities who reside within the five boroughs of New York City. Our programs include Educational Services, Adult Day Services, Vocational Services and Residential Services, as well as Medicaid Service Coordination and Clinical Services.

    Cardinal Hayes Home has one hundred ten developmentally disabled young people residing in its Intermediate Care Facilities (ICFs). The main campus in Millbrook, NY is home for sixty children and young adults. There is 24-hour care and supervision of the residents at every agency site.

    We accept children who are:

    •age 16 and under at time of admission
    •ambulatory and non-ambulatory
    •medically fragile, but stable
    •tube-fed or on special diets
    •in need of ICF level of care placement

    We are not able to accept children:
    •requiring 24-hour skilled nursing care
    •with tracheotomies
    •under the age of 2
    •requiring palliative or hospice care on admission

    Pediatric residences are home to children from the age of 5 to 21. The Center’s beautiful pediatric campus is shaped by this ideal, and includes an outdoor learning environment, creative classroom spaces and comfortable residences. On-site medical, physical, and psychological support is administered through clinics for primary medicine, dental services, neurology, orthopedics, psychiatry, podiatry and many more specialties.
