This resource directory is for families/caregivers of children who have medically complex needs or long-lasting conditions that require extra health care and support services.

The information in this directory is provided to help you find resources to assist you with finding help for your child and family. The range of care/disability/health issues varies amongst children so the information may or may not pertain to your child.

After a family learns that their child is ill parents/caregivers typically begin to think about how they can best care for their child. The resources in this directory include agencies, service providers, advocacy organizations, and other resources to assist with providing a quality life for children with medically complex needs.

    The Staten Island Center for Independent Living, Inc., is a non-residential resource center that offers services to individuals with disabilities of all ages, their families, significant others, teachers, employers, businesses, and those who believe that an individual with a disability has the right to take responsibility for his/her own life.

    The center is unique in that it is primarily staffed and governed by professionals who have had a personal experience with a disability and believe that disability does not mean inability.

  6. Lifeline- discount phone service usac.org

    Lifeline is a government benefit program that provides discounts on monthly telephone service for eligible low-income subscribers to help ensure they can connect to the nation’s communications networks, find jobs, access health care services, connect with family and their children’s schools, and call for help in an emergency. Lifeline is supported by the federal Universal Service Fund (USF).

    Lifeline provides discounts on monthly telephone service (wireline or wireless) for eligible subscribers. These discounts are currently set at $9.25 per month. Federal rules prohibit eligible low-income subscribers from receiving more than ONE Lifeline service per household.

  8. Medicaid Waiver Services

    Waiver services allow people with disabilities to receive long-term care services in most integrated setting in their communities, rather than receiving care in an institutional setting.  These programs “waive” or remove some of the requirements for getting Medicaid services.  There are a variety of Medicaid waivers that serve children and/or young adults, and there are specific eligibility requirements, guidelines & rules, funding limitations and participation limits for each of New York State’s current waivers.  Generally, the cost of receiving services through a waiver needs to be less than the cost of services if the individual were placed in a nursing home or other institutional setting.

    Waiver programs for children in NYS include:

    Bridges to Health (B2H)– provides certain services to children in foster care who have mental health, developmental disabilities or health care needs to help them live in the community.  Office of Children and Family Services: 518-408-4064 or Department of Health: 518-486-6562      www.ocfs.state.ny.us/main/b2h

    Care at Home (CAH)- provides medical assistance to families with children living at home with severe disabilities or medical conditions.  There are different levels of CAH.  Contact local CAH coordinator at local Developmental Disabilities Services Office.    www.opwdd.ny.gov

    Home and Community Based Services (HCBS)- provides services and support to children and adolescents with serious emotional disturbances to remain at home.   www.opwdd.state.ny.us

  10. NYC Care at Home Program nyc.gov

    The Care-At-Home (CAH) Program provides a Medicaid-financed plan of community-based services to enable severely physically disabled children – who might otherwise qualify for hospital or nursing home placement – to live with th eir families. All Medicaid and additional services, such as case management and home adaptations, are available to assist a family in caring for their child.

    The Care at Home Program includes a Medicaid waiver enabling parents normally not eligible to qualify their child for Medicaid.
