This resource directory is for families/caregivers of children who have medically complex needs or long-lasting conditions that require extra health care and support services.
The information in this directory is provided to help you find resources to assist you with finding help for your child and family. The range of care/disability/health issues varies amongst children so the information may or may not pertain to your child.

After a family learns that their child is ill parents/caregivers typically begin to think about how they can best care for their child. The resources in this directory include agencies, service providers, advocacy organizations, and other resources to assist with providing a quality life for children with medically complex needs.

The directory is intended to assist you with being the best advocate for your child. Although we attempt to keep the directory current resources change constantly.

NYC Department of Homeless Services- Client Advocacy nyc.gov
The Department of Homeless Services’ (DHS) Office of Client Advocacy (OCA) is led by highly trained and professional workers with extensive experience providing services to homeless families and individuals. Tailoring assistance toward the individual needs of clients, OCA can help households navigate the shelter system, learn about their rights and responsibilities, and resolve conflicts/concerns by liaising between clients and shelter staff.

NYC Housing Authority (NYCHA) nyc.gov
The New York City Housing Authority (NYCHA)’s mission is to increase opportunities for low- and moderate-income New Yorkers by providing safe, affordable housing and facilitating access to social and community services. More than 400,000 New Yorkers reside in NYCHA’s 334 public housing developments around the five boroughs.

NYC Housing Connect- Department of Housing Preservation and Development nyc.gov
Assists with finding and applying for affordable housing, HPD does not rent apartments. We work with real estate professionals and community sponsors who market apartments. We require that subsidized apartments be rented through an Open Lottery System to ensure fair and equitable distribution of housing to eligible applicants.
In order to participate in housing lotteries, you will have to contact building managers, community sponsors and/or real estate professionals directly to fill out an application and enter it in an apartment lottery. There are no brokers’ fees and no application fees.

NYC Rent Assistant Programs nyc.gov
A variety of government agencies and nonprofit organizations provide support to those who have fallen behind on rent

Project Open House- Mayors Office of People with Disabilities NYC nyc.gov
Provides funding for the removal of architectural barriers in the homes of people with permanent disabilities
All applicants must submit the Registration Form to MOPD no later than September 15th, 2014. Applicants are urged to submit the application via the online Registration Form.
This program is on a first come, first served basis. Late applications will not be accepted, and submission of an application does not guarantee acceptance.

Public Housing Information and Resource Center (PIH) nyc.gov
Provides decent and safe rental housing for eligible low-income families and persons with disabilities. PIH Information/Resource Center is staffed to answer questions/inquiries regarding public housing and housing choice voucher programs and regulations.

Sinergia- Tier II Shelter and Housing Advocacy sinergiany.org
Sinergia, Inc. operates a Tier II Program, which is transitional housing for homeless families who have members that are developmentally disabled. It is operated under the Division of Homeless Services (DHS).

The Bridge Fund thebridgefund.org
Bridge Fund assists to prevent homelessness for families who are threatened with the loss of housing, but often do not qualify for emergency governement assistance.
All residents of New York City boroughs must be referred by another agency to The Bridge Fund.

The Queens Community House queenscommunityhouse.org
Provides programs and services which help people improve their lives. Provides housing assistance, financial literacy workshops, and citizenship classes. Bengali, Chinese, French, Pashto, Russian, Spanish. Do not have to be a documented immigrant to receive services.
